The biggest shift in tooth replacement philosophy over the past two decades has been a deepening understanding of what is actually lost when a tooth is extracted. The visible crown above the gumline is only part of the story - below the surface, the tooth root was actively stimulating the surrounding jawbone every time it encountered biting force. Remove the root, and that stimulation stops. Bone resorption after tooth loss proceeds steadily, sometimes dramatically, changing the contour of the jaw and the way the face is supported over years.

This biological reality makes dental implants - which replace the root, not just the crown - the most anatomically faithful tooth replacement option available. But implants are not the right answer for every patient, and at Shree Hospitals, our approach to full mouth rehabilitation and tooth replacement care always begins with an honest, individual assessment of what each patient needs, what their bone and gum tissue can support, and what their functional and aesthetic priorities are.

1. Comprehensive Pre-Treatment Assessment

• Full mouth clinical and radiographic examination - assessing the condition of remaining teeth, gum tissue health, and available bone volume
• CBCT (Cone Beam CT) three-dimensional imaging - measuring bone height, width, and density at potential implant sites with submillimetre accuracy
• Occlusal analysis - understanding how the remaining teeth bite together and how the replacement tooth will integrate into the overall bite
• Medical history review - identifying any conditions affecting implant eligibility (uncontrolled diabetes, anticoagulant therapy, bisphosphonate use)
• Discussion of all available fixed vs removable teeth options with a clear explanation of the advantages, limitations, and costs of each

2. Dental Implant Treatment
    
   The gold-standard tooth replacement for most patients:

• Titanium implant fixture placement into the jawbone under local anaesthesia - a straightforward, minimally invasive procedure for most patients
• Osseointegration period of 3 to 6 months - during which the implant fuses with the surrounding bone
• Abutment placement and custom-fabricated ceramic crown delivery - restoring the complete tooth from root to visible crown
• Immediate loading protocols where bone quality permits - provisional crown placed the same day as implant surgery
• All-on-4 and All-on-6 full arch implant protocols for completely edentulous patients - four or six strategically placed implants supporting a full arch of fixed teeth

3. Bone Grafting for Insufficient Bone Volume
    
   Bone resorption after tooth loss often leaves insufficient bone for implant placement:

• Socket preservation grafting at the time of extraction - preventing the inevitable bone resorption that follows tooth removal
• Lateral ridge augmentation using block grafts or particulate bone mixed with collagen membrane
• Sinus lift augmentation for the posterior upper jaw - where the sinus floor encroaches on available implant height
• Vertical bone augmentation for severely resorbed ridges - the most complex grafting procedure, requiring careful planning and timing

4. Fixed Bridge Alternatives
    
   For patients who cannot or prefer not to have implant surgery:

• Tooth-supported ceramic or zirconia bridge - crowning the adjacent teeth to support a false tooth in the gap
• Maryland (resin-bonded) bridge for selected single tooth gaps - minimal preparation of adjacent teeth required
• Clear explanation that bridges do not prevent bone resorption after tooth loss - this remains a significant long-term consideration

5. Removable Prosthetic Options
    
   Where implants are not clinically appropriate:

• Acrylic partial denture - an affordable removable option for multiple missing teeth
• Cast metal partial denture - providing greater rigidity and stability than acrylic alone
• Implant-retained overdenture - a removable prosthesis supported and stabilised by two or four implants - dramatically improving stability, retention, and quality of life compared to conventional dentures
• Complete denture for fully edentulous patients - with thorough impression techniques and balanced occlusion for optimal comfort and function

6. Full Mouth Rehabilitation
    
   For patients with multiple missing teeth or significant functional compromise:

• Comprehensive treatment planning addressing all missing and compromised teeth simultaneously
• Staged treatment coordinating implant placement, bone grafting, and prosthetic delivery in a logical sequence
• Aesthetic wax-up and mock-up before any irreversible treatment - showing the patient the expected outcome
• Functional bite analysis ensuring the complete rehabilitation works harmoniously throughout all jaw movements
