Not every wisdom tooth needs to be removed - this is an important starting point that distinguishes thoughtful care from reflexive extraction. A wisdom tooth that has erupted fully into a functional position, can be adequately cleaned, and is not causing problems to itself or adjacent teeth may be left in place. The decision to remove should be based on clinical indication, not convenience or assumption.

That said, the majority of wisdom teeth presenting to our clinic do have clinical indications for removal - recurrent partial impaction and gum infection (pericoronitis), decay in the wisdom tooth or the adjacent second molar, cyst development, or straightforward lack of space to erupt. When removal is indicated, our approach at Shree Hospitals is built around thorough pre-surgical planning, predictable surgical technique, and clear post-operative support that makes recovery as straightforward as possible.

1. Clinical & Radiographic Assessment

• Clinical examination - assessing eruption status, gum pocket depth around the wisdom tooth, and any signs of partial impaction and gum infection
• Dental panoramic X-ray - showing the position, angulation, and depth of all four wisdom teeth simultaneously alongside their relationship to the inferior alveolar nerve
• CBCT (Cone Beam CT) imaging for lower wisdom teeth in close proximity to the inferior alveolar nerve - providing three-dimensional spatial relationship data that significantly reduces nerve injury risk
• Classification of impaction type - mesioangular, distoangular, vertical, or horizontal impaction each requiring different surgical approaches
• Assessment of root morphology - fused, divergent, or hooked roots each affecting difficulty and extraction technique

2. Conservative Management When Appropriate
    
   For selected cases not immediately requiring extraction:

• Third molar pain relief and pericoronitis treatment with irrigation, antibiotics, and operculum removal for recurrent pericoronal infections
• Monitoring with annual panoramic radiography for asymptomatic fully impacted teeth where extraction risk is significant
• Honest discussion of observation versus prophylactic removal - respecting patient autonomy with accurate information about risks and benefits of each approach

3. Surgical Wisdom Tooth Extraction
    
   Impacted wisdom tooth extraction and surgical care in Mumbai is performed with meticulous pre-operative planning:

• Amoxicillin and metronidazole antibiotic combination for patients with significant infection or systemic signs

• Emergency management for patients with significant trismus or spreading cellulitis

5. Nerve Protection Techniques
    
   Lower wisdom tooth extraction carries specific risks to the inferior alveolar and lingual nerves:

• Coronectomy technique - deliberate removal of the crown only, leaving roots in situ for teeth where roots are in direct contact with the nerve canal on CBCT imaging
• Careful surgical technique avoiding lingual retraction that risks lingual nerve injury
• Cone beam CT-guided planning for close proximity cases - identifying the most nerve-protective surgical approach
• Transparent pre-operative consent including specific nerve injury risk discussion based on individual radiographic findings

6. Post-Operative Care & Recovery
    
   Wisdom tooth surgery recovery is optimised through clear, specific aftercare:

• Bite on gauze for 45 minutes immediately post-surgery to control initial bleeding
• Cold compress application for the first 24 hours - reducing swelling significantly
• Soft diet for the first 3 to 5 days, avoiding hot drinks and foods
• Chlorhexidine mouthwash from day two - maintaining hygiene without disturbing the healing clot
• NSAIDs and paracetamol prescription for pain management - most patients find adequate relief with these alone
• Trismus (limited mouth opening) is expected for a few days and resolves gradually
• Review appointment at 7 days for suture removal and healing assessment
