1. Thorough Diagnostic Evaluation

• Transthoracic and transoesophageal echocardiography (TEE) to assess valve structure and severity
• Cardiac MRI for detailed tissue characterisation and functional assessment
• Cardiac CT angiography for pre-procedural planning of transcatheter valve procedures
• Exercise stress testing to unmask symptoms in patients with apparently stable disease
• Cardiac catheterisation to evaluate haemodynamics and coronary anatomy before surgery

2. Medical Management & Watchful Waiting

    

   Not every valve abnormality requires immediate intervention. For mild to moderate disease:

• Diuretics and vasodilators to reduce cardiac workload and manage fluid retention
• Rate control medications for patients with valve stenosis and regurgitation complicated by atrial fibrillation
• Anticoagulation therapy to prevent clot formation on diseased valves
• Structured surveillance with repeat echocardiography at defined intervals to track progression

3. Surgical Valve Repair, Preserving What's Natural

    

   Where feasible,  our structural heart specialists prefer valve repair over replacement, preserving the native tissue for better long-term outcomes:

• Mitral valve repair using annuloplasty rings,  leaflet resection,  or chordal reconstruction
• Tricuspid valve repair for functional regurgitation associated with left-sided disease
• Aortic valve repair in selected cases of aortic regurgitation with preserved leaflet tissue
• Minimally invasive approaches through smaller incisions for faster recovery and reduced hospital stay

4. Surgical Valve Replacement

    

   When repair is not feasible,  valve replacement restores normal cardiac function effectively:

• Mechanical valve replacement, durable and long-lasting,  requiring lifelong anticoagulation
• Biological (tissue) valve replacement, natural feel,  preferred in elderly patients avoiding blood thinners
• Ross procedure for younger patients requiring aortic valve replacement
• Full sternotomy or minimally invasive right mini-thoracotomy depending on patient anatomy

5. Transcatheter Valve Interventions

• TAVR (Transcatheter Aortic Valve Replacement) for severe aortic stenosis in high or intermediate surgical risk patients
