Cervical Cancer

Cervical cancer is one of the most preventable and when detected early most treatable cancers affecting women. The vast majority are caused by persistent infection with human papillomavirus (HPV). At Shree Hospitals, our gynaecologic oncology team brings together surgical expertise, radiation, and systemic therapy for comprehensive cervical cancer treatment in Mumbai India, backed by a deep commitment to early detection through Pap smear screening in Mumbai.

(24×7 Emergency Care)

Our Approach to Managing Cervical Cancer

Cervical cancer is unique among gynaecologic cancers in that it is almost entirely preventable with vaccination and screening yet it remains one of the leading causes of cancer mortality in Indian women. At Shree Hospitals, our cervical cancer specialists in Mumbai treat the full spectrum: from pre-invasive lesions (CIN) managed conservatively to locally advanced disease requiring multimodal treatment.

1. Diagnosis & Staging

  • Pap smear and colposcopy first steps in evaluating abnormal cervical cells
  • Colposcopy-directed biopsy for histological confirmation
  • MRI pelvis to assess tumour size, parametrial involvement, and nodal disease
  • CT chest and abdomen for distant staging
  • Examination under anaesthesia (EUA) for accurate clinical staging in advanced cases

2. Surgery for Early-Stage Cervical Cancer

Surgery is the treatment of choice for Stage I and early Stage II disease:

  • Radical hysterectomy (Wertheim's) with bilateral pelvic lymph node dissection
  • Laparoscopic or robotic radical hysterectomy minimally invasive options for eligible patients
  • Trachelectomy fertility-preserving surgery for young women with Stage IA2–IB1 disease
  • Sentinel lymph node mapping to reduce the extent of lymph node surgery
  • Ovarian conservation offered to pre-menopausal women where oncologically safe

3. Definitive Chemoradiation

For locally advanced cervical cancer (Stage IB3 and beyond), concurrent chemoradiation is the standard of care:

  • External beam radiation to the pelvis IMRT technique for bowel and bladder sparing
  • Weekly cisplatin chemotherapy radiosensitiser given throughout external beam course
  • Treatment duration approximately 5–6 weeks
  • High-Dose Rate (HDR) Brachytherapy as the final boost delivers high focal doses to the cervix

4. HDR Brachytherapy

Brachytherapy is the component that makes cervical cancer chemoradiation curative:

  • Image-guided adaptive brachytherapy (IGABT) MRI-guided planning for precision
  • Applicators placed under anaesthesia; treatment delivered in 3–5 fractions
  • Dramatically reduces residual tumour burden in the cervix and parametria
  • Critical component of HPV-related cancer care and definitive treatment

5. Systemic Therapy for Metastatic Disease

Advanced or recurrent cervical cancer is managed with:

  • Carboplatin-paclitaxel with bevacizumab standard first-line regimen
  • Pembrolizumab (immunotherapy) added for PD-L1-positive tumours now standard of care
  • Tisotumab vedotin for recurrent, heavily pretreated cervical cancer
  • Palliative radiation for symptom control haemostasis, pain, obstruction

6. HPV Prevention & Survivorship

Prevention is the most powerful tool in cervical cancer:

  • HPV vaccination recommended for girls aged 9–26 and up to 45 in selected cases
  • Regular Pap smear screening in Mumbai every 3 years for women 21–65 (or HPV co-testing every 5 years from age 30)
  • Post-treatment surveillance: clinical exam, Pap smear, and imaging at defined intervals
  • Lymphoedema management, sexual health counselling, and psychological support

How To Identify Cervical Cancer?

Early cervical cancer is usually asymptomatic detected only through screening. This is why Pap smear screening in Mumbai is non-negotiable for sexually active women. When symptoms appear, they include:

  • Abnormal vaginal bleeding — between periods, after intercourse, or post-menopausal bleeding
  • Unusual vaginal discharge — watery, blood-tinged, or foul-smelling
  • Pelvic pain — persistent pain in the lower abdomen or pelvis not linked to menstruation
  • Pain during sexual intercourse — dyspareunia that is new or worsening
  • Lower limb swelling — suggests lymph node involvement in the pelvis
  • Urinary or bowel symptoms — difficulty passing urine or blood in urine in advanced disease

Any woman experiencing these symptoms should consult a cervical cancer specialist in Mumbai at Shree Hospitals immediately, and all women should ensure they are up to date with their Pap smear.

Important FAQs: Cervical Cancer

Is cervical cancer caused by HPV?

In over 99% of cases, yes. HPV particularly types 16 and 18 is the causative agent. However, most women who get HPV never develop cervical cancer. The cancer develops only in those with persistent infection who don't clear the virus naturally.

At what age should I start getting Pap smears?

At 21, or within 3 years of first sexual activity — whichever comes first. Women 21–29 should have a Pap smear every 3 years. From age 30, HPV co-testing every 5 years is preferred. Speak to our team for personalised guidance.

Can cervical cancer be prevented?

Yes, to a large extent. HPV vaccination (ideally given before sexual debut) and regular Pap smear screening dramatically reduce the risk of developing cervical cancer. These two measures together could eliminate the disease as a public health problem.

Will I be able to have children after treatment?

If detected at an early stage, fertility-preserving surgery (trachelectomy) may be possible. For women who need radical hysterectomy or radiation, pregnancy is not possible. Our team discusses all options including fertility preservation before chemotherapy at the time of diagnosis.

What is the success rate of chemoradiation for cervical cancer?

For Stage II disease, 5-year overall survival with chemoradiation and brachytherapy is approximately 60–75%. Stage I has even higher cure rates with surgery. Outcomes depend significantly on stage, tumour volume, and quality of brachytherapy.

Does cervical cancer treatment affect sexual health?

It can. Radiation may cause vaginal dryness, stenosis, and reduced lubrication. Surgery may affect the nerve supply to the vagina. Our team provides proactive sexual health counselling and recommends vaginal dilator therapy post-radiation to minimise long-term effects.

Treatments For Cervical Cancer at Shree Hospitals

We offer a complete range of cervical cancer treatment services, including advanced screening and diagnosis, personalised treatment plans, minimally invasive and fertility-preserving surgery where appropriate, radiation therapy, chemotherapy, targeted therapy, immunotherapy, rehabilitation, and long-term survivorship care designed to help patients achieve the best possible outcomes.

Radical Hysterectomy

Surgical removal of the uterus, cervix, upper vagina, and surrounding tissue along with pelvic lymph nodes. Performed laparoscopically or open depending on disease extent. Curative for early-stage cervical cancer treatment in Mumbai India.

Radical Trachelectomyz

Fertility-preserving surgery that removes the cervix while leaving the uterus intact. Offered to young women with small, early-stage tumours. Allows future pregnancy with appropriate obstetric monitoring.

Concurrent Chemoradiation

The standard treatment for locally advanced disease, external beam pelvic radiation with weekly cisplatin. Followed by HDR brachytherapy boost. Curative in the majority of patients when delivered with precision.

HDR Brachytherapy

Internal radiation delivered directly to the cervix and surrounding tumour. Image-guided adaptive planning ensures maximum tumour dose with minimal normal tissue exposure a critical component of HPV-related cancer care.
